The most recent secondary <br />containment testing, the routine monitoring system certification reports, and alarm logs for all alarms (some alarms <br />were documented and the following were not: L2 on 7-1-16, L4 on 5-6-16, L5 on 4-9-16) couldn't be located at the <br />time of inspection These records shall be maintained on site for at least three years. Monitoring records include: (1) <br />date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) <br />results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all <br />readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for <br />the last three years on site and submit copies to the EHD. <br />This is a Class II violation. <br />205 <br />CCR 2712(i) Failure to have an approved UST response plan available onsite. <br />The response plan was not available onsite at the time of inspection. Ensure a copy is available for inspection at any <br />time. Submit proof of correction to the EHD. <br />This is a Class II violation. <br />210 <br />CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />All 2016 designated operator monthly inspection reports were not found on site. The issue was brought to the <br />attention of Mr. Rattu, who also looked for the missing reports and was unable to locate them. Designated operator <br />monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of <br />the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies <br />of all missing reports to the EHD. <br />This is a Class II violation. <br />301 <br />HSC 25292.1 UST system is not operated to prevent unauthorized release, including spills and overfills. <br />Leaking pipe union was observed above the shear valve on the 87-octae product piping inside UDC-1/2. The <br />underground storage tank system shall be operated to prevent unauthorized releases. Immediately make necessary <br />repairs, submit proof of correction to the EHD. <br />This is a Class II violation. <br />315 <br />HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br />Water was found in the 91 STP, 91 Fill, and 87 fill sumps. Secondary containment shall be constructed to prevent <br />any water intrusion into the system by precipitation, infiltration, or surface runoff. The service technician removed <br />3-4 gallons of liquid from fill sumps and <1 gallon from the <91 STP sump, and returned the sensor to its proper <br />location. Ensure that all sumps and annular spaces are maintained free of liquid. This was corrected at the time of <br />inspection. <br />This is a repeat violation, Class II. <br />Page 4 of 5 <br />
