<br />SUMMARY OF VIOLATIONS
<br />CLASS I, CLASS II, or MINOR -Notice to Comply)
<br />This is a Routine and Monitoring Certification inspection. The following are Class II Repeat violations:
<br />#104, 204, 404, 405, and the following is a Minor -Notice to Comply: #201, 403
<br />104. Financial responsibility documents have not been submitted to the EHD. Current financial
<br />responsibility documents are required to be submitted annually. Complete and submit a copy of the
<br />financial responsibility documents. A copy of the current financial repsonsibility documenta has been
<br />provided to EHD during this inspection. Violation was corrected on site.
<br />201. Volume and type of brine added in February, March, April, and July were not documented. The
<br />volume , location, and type of brine added to the secondary containment shall be documented as a part
<br />of your record maintenance, and to evaluate if the loss of brine indicates a compromise to the secondary
<br />containment system. Immediately begin documenting the location, volume, and type of brine when
<br />adding to the secondary containment.
<br />204. The designated operator failed to check and note the L12: Dispenser #3/4 pan fuel alarm on
<br />February 28, 2012 in the March 23, 2012 -designated operator monthly inspection report. During the
<br />monthly inspection, the designated operator shall check that all required testing and maintenance for the
<br />UST system have been completed and document the dates they were done. Ensure that designated
<br />operators performing monthly inspections at this facility are including all of the required information on
<br />the reports.
<br />403. About 12 oz of brine had to be added in 91 octane fill annular reservoir for the sensor to come out
<br />of alarm after testing, and unknown volume of brine was added in Dispenser #1 /2 and 3/4 brine
<br />reservoirs in the months of March/April/July, and February/March 2012, respectively due to fuel alarms,
<br />indicating a leak in the secondary containment. Secondary containment shall be impervious to the liquid
<br />and vapor of the substance contained and constructed to prevent structural weakening as a result of
<br />contact with any hazardous substances released from the primary containment. Immediately contact a
<br />properly licensed, trained, and certified contractor to make repairs to the UST system under permit and
<br />inspection of the EHD.
<br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
<br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE.
