Also SR0060156 cold start inspection. <br />4. Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility by November 8, 2010. <br />7. The following maintenance receipts were not found on site. The following monitoring system alarms <br />do not have service receipts for work done to correct the alarm condition. July 23, 2010 setup data <br />warning alarm for all sensors, L11 - fuel alarm Sept 22, 10, L12 - fuel alarm Sept 14, 10, L13 - fuel <br />alarm Sept 22, 10, L14 - fuel alarm Sept 22, 10, S1 - communication alarm = Sept 1, 10, S2 - <br />communication alarm - Sept 8, 10, L1 - setup data warning August 20, 2010. These records shall be <br />kept on site for at least 3 years. Immediately locate and maintain the service receipts on site and submit <br />copies to the EHD by November 8, 2010. The EHD recommends that service receipts be attached to <br />the monthly designated operator reports. <br />44. Dispenser 5/6 is losing brine and was in alarm during today's inspection. The dispenser sump was <br />dry suggesting that the brine is leaking into the backfill outside the dispenser sump. Secondary <br />containment shall be impervious to the liquid and vapor of the substance contained and constructed to <br />prevent structural weakening as a result of contact with any hazardous substances released from the <br />primary containment. Immediately contact a properly licensed, trained, and certified contractor to make <br />repairs to the UST system under permit and inspection of the EHD.
