The 91 fill sump sensor was raised approximately four inches off the lowest point of the <br /> sump and not located to detect a leak at the earliest opportunity. A large amount of liquid was observed <br /> in the sump. Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br /> possible opportunity. The repair technician removed liquid from the sump and placed the sensor at the <br /> low point of the sump. <br /> 403, 406. The following was found in the tank pad sumps indicating leaks in the double walled sumps <br /> and lack of monitoring of the sump interstitial space: <br /> a. 91 product turbine sump: broken penetration fittings, plugged interstitial fluid hoses and no <br /> interstitial fluid in the interstice of the double walled sump. <br /> b. 91 fill sump: interstitial fluid inside the sump, cracked penetration fitting. <br /> c. 87 fill sump: interstitial fluid inside the sump, leaking penetration fitting. <br /> The double walled sumps monitoring system shall be maintained so that a breach in the primary or <br /> secondary containment is detected before the hazardous substance is released into the environment. <br /> Immediately contact a properly licensed, trained, and certified contractor to make repairs to the UST <br /> system under permit and inspection of the EHD. If the failed components can't be repaired <br /> immediately, there is a possibility that the UST system may be red tagged by the EHD to prevent fuel <br /> inputs. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector: Received By: Title: <br /> E Rev 04/19/12 Page 3 UST INSPECTION REPORT <br />
