QoPQU1Nci SAN4J0A0UIN <br />COUNTY <br />``ql,FO P, Greatness grows here. <br />August 30, 2018 <br />Elite IV Contractors <br />Attn: Megan Mitchell <br />2535 Wigwam Dr. <br />Stockton, CA 95205 <br />Re: Waterloo Food Mart <br />4315 E Waterloo Rd <br />Stockton, CA 95215 <br />Environmental Health Department <br />Linda Turkatte, REHS, Director <br />Kasey Foley, REHS, Assistant Director <br />PROGRAM COORDINATORS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Rodney Estrada, REHS <br />Willy Ng, REHS <br />Muniappa Naidu, REHS <br />B FILE copy <br />Amended- SR0078796 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br />amended application for a permit to (1) break concrete around 87 submersible turbine pump (STP) <br />sump, 91 STP sump, vent sump, and all dispensers, (2) replace all penetrations in 87 STP and 91 STP <br />sumps and vent sump with Bravo F -series split repair penetrations, (3) install Western Fiberglass test <br />port in 87 STP sump (4) install new water tight manways at 87 STP and 91 STP sumps, (5) remove all <br />existing under dispenser containments (UDCs) and replace with Western Fiberglass double wall UDCs <br />with Smith fiberglass entry fittings and new Franklin Fueling Systems flexible connectors, (6) replace <br />both submersible turbine pumps and reinstall the two existing line leak detectors, (7) install new <br />Wayne Ovation dispensers, (8) perform a monitoring system cold start on the underground storage <br />tank (UST) system at the above noted address. The permit has been approved with the following <br />conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control District and <br />local city building department for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />3. Contact Stacy Rivera at (209) 468-3440 to arrange the following inspection appointments (a <br />48-hour notice is required for inspection appointments). Please be aware that inspections for <br />emergency work must be scheduled within five days of permit approval. <br />a. Witness soil sampling from under each removed UDC and piping. Since primary piping is <br />removed outside of UDC, additional soil sampling below removed piping will be required <br />based on field observations and as outlined in the sampling protocol. Analytical results to <br />be provided to Environmental Health Department. The San Joaquin County Sampling <br />Protocol For Routine Piping/Dispenser Removal /Replacement shall be followed. <br />b. Primary piping soap test on all installed piping and penetrations: One-hour pneumatic <br />pressure test at 110% of operating pressure, or 40 psi, whichever is greater. <br />c. Secondary piping slope (at 1/8 inch per foot or greater) of installed piping to a containment <br />sump with an audible/visual alarm. Verification of appropriate backfill, piping setbacks, and <br />crossovers. <br />d. Secondary piping soap test on all installed piping and penetrations: One-hour pressure test <br />at the manufacturer's recommended specification. <br />1868 E.
