Box 31325 <br /> Stockton,CA 95213 <br /> RE: Waterloo Shell,4315 Waterloo Road,Stockton(SR0057244) <br /> The San Joaquin County Environmental Health Department(EHD)has completed the review of <br /> the application for a permit to install a Veeder Root 323 liquid sensor in the 91 turbine sump at <br /> the above noted address. The sensor was installed on May 6,2009 and tested during the <br /> monitoring system certification on May 6, 2009. The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council(ICC),and <br /> c. A valid, current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired, or serviced,and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 3. Permit expires 180 days from date of approval. Note: If an extension to the expiration <br /> date is required, a written request must be submitted to this office prior to the expiration <br /> date. <br /> 4. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API, ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 5. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 6. A"Certification of Installation/Modification" shall be submitted by the owner or the <br /> owners'agent [pursuant to(CCR) 2635(d)] before this repair will be considered complete <br /> by the EHD. <br /> Page 1 of 2 <br />
