7474 Eleventh Street Chronology <br /> Page 2 <br /> 04/13/98 Groundwater monitoring report. All monitoring wells and drinking water <br /> well sampled 3/18/98. No impact to drinking water well. Low levels of <br /> contamination in water from MW1, MW2, MW101. Report recommends <br /> no further action or monitoring. <br /> 08/05/99 PHS/EHD received Addendum to Groundwater Monitoring Report dated <br /> 8/31/98. Site closure check list from Appendix B of the Tri Regional <br /> Guidelines discussed. Mass remaining estimated to be 507 gallons of <br /> gasoline. <br /> 09/16/99 PHS/EHD letter requiring more complete survey of wells within 2000 feet <br /> radius of site, drinking water well be destroyed, and a new well installed <br /> away from contamination left in place. <br /> 12/22/99 Work plan submitted for well survey of wells within 1000 feet of the site, <br /> video of drinking water well, destruction of monitoring wells and drinking <br /> water well and installation of a new drinking water well. <br /> 02/01/00 PHS/EHD letter requiring 2000 feet radius well survey with visual <br /> identification of wells since many were installed prior to DWR well logs <br /> being required. <br /> 04/11/00 PHS/EHD letter stating monitoring wells are not to be destroyed until <br /> CVRWQCB concurs with site closure. <br /> 06/26/00 Video of drinking water well performed. <br /> 08/10/00 2000 feet radius well survey submitted. On site drinking water well is the <br /> closest. Next closest wells are about 500 feet away. Groundwater <br /> contamination defined to be on site. <br /> 08/30/00 Discussed site with CVRWQCB staff who requested current sampling <br /> and analysis of water from all wells. Informed responsible party and <br /> consultant of this requirement by telephone. <br /> 08/31/00 Monitoring wells and drinking water well water samples collected. <br /> 09/13/00 New drinking water well installed 150 feet east of the contamination. <br /> 09/19/00 Another water sample collected from MW 1 and MW 101. <br /> 10/11/00 Report of monitoring well water sample results. Water samples from <br /> MW 1 and MW2 have low levels of contamination. <br /> 11/01/00 PHS/EHD concurs with closure of site. Old drinking water well is <br /> destroyed. <br />
