3. PUBLIC HEALTH SERVICES: (Contact Person: Carol Oz, 468-3440) <br /> a. A soil suitability study, as required by Development Title Section 9-1105.2(d), which <br /> indicates that the area is suitable for a septic tank system must be performed prior to <br /> recordation of the Parcel Map. A review fee of$156.00 is required at the time of submittal <br /> to the Environmental Health Division. <br /> b. Construction of an individual sewage disposal system under permit and inspection by the <br /> Environmental Health Division is required at the time of development. (Development Title <br /> Sections 9-1110.3 and 9-1110.4) <br /> C. Construction of an individual domestic water well under permit and inspection by the <br /> Environmental Health Division is required at the time of development. (Development Title <br /> Section 9-1115.3) <br /> d. The existing private water wells shall be tested for the chemicals dibromochloropropane <br /> (DBCP) and nitrates prior to recordation of the Parcel Map. Samples are to be taken and <br /> analyzed by a State-approved laboratory. (Development Title Section 9-1115.7) <br /> 4. LODI UNIFIED SCHOOL DISTRICT: <br /> a. Pursuant to that separate Memorandum of Understanding between the Developer and the <br /> Lodi Unified School District (the 'District') relating to the Project, Developer and District <br /> have agreed that the following condition be recorded on the map for the Project: The <br /> Developer shall pay to the District the sum of$21,369.15 prior to recording the final map <br /> creating the lots within the Project. Such sum is computed upon the basis of a per unit <br /> fee to the District of$7,123.05 for each of the units to be constructed upon the three new <br /> lots within the Project. Under the terms of the afore-mentioned MOU, upon receipt of the <br /> above-described fee by the District, the District shall prepare and submit a written <br /> certification of receipt to the Community Development Department, Development Services <br /> Division of San Joaquin County. District shall also provide to Developer a release, in a <br /> form suitable for recording, establishing satisfaction of the afore-described condition. In <br /> addition, any second units constructed on lots within the Project created by way of the <br /> recording of a subsequent map, or by way of lot split, lot line adjustment, or similar <br /> developmental approval will be subject to a school facility fee to the District in the amount <br /> of $2.91 per square foot up to a maximum fee of $7,123.05 payable at time of building <br /> permit issuance for such unit. The within condition constitutes a condition of approval <br /> for the Minor Subdivision Map for the Project. <br /> Conditions of Approval - 3 - MS-94-51 <br />
