Glover <br /> Project Number: 95-122-01 <br /> General Work Description: Installation of 3 groundwater monitoring wells and <br /> r sampling. <br /> L <br /> SSP Prepared by: William Madison <br /> On-site Safety Officer: William Madison <br /> Project Safety Officer: John Lane <br /> This site safety and health plan (SSP) must be kept on-site and accessible to all on-site <br /> employees during all phases of site operations. The SSP must be reviewed and signed by <br /> all on-site employees, subcontractors, and visitors prior to entering the site. General safe <br /> work practices are identified in the Smith Environmental Technologies Corporation (Smith <br /> Environmental) Injury and Illness Prevention Manual and apply to all Smith Environmental <br /> work sites. <br /> CTheP rovisions set-forth in this SSP shall apply to Smith Environmental employees and any <br /> subcontractors working for Smith Environmental at the job site. All personnel working for <br /> Smith Environmental at the job site must`read this SSP, and sign the attached Compliance <br /> Agreement before entering the work area. Field personnel may deviate from the safety <br /> provisions set-forth in this SSP, but only to upgrade or increase the safety requirements. <br /> Smith Environmental personnel may suspend work if unsafe working conditions are <br /> identified or observed, or unauthorized modifications to the safety provisions set-forth in <br /> this SSP are made. If changes in site or working conditions require changes in safety <br /> procedures, appropriate amendments to this SSP will be provided by the Smith <br /> Environmental project manager or supervisor, with the written approval of a Smith <br /> Environmental branch safety officer or the Smith Environmental Health and Safety <br /> Director. <br /> L <br />
