The Order requires <br /> the dairy owner or operator to file annual reports and to maintain specific records (see the enclosed "Information <br /> :Sheet For Dairies Under General Requirements"). <br /> a <br /> Our records indicate that we have not received the required annual reports for your dairy for at least the past year <br /> I (the report due 30 November 1998). Accordingly,you are being sent this Notice of Violation (NOV). Failure to <br /> submit the required report is subject to enforcement action that may include civil fines of up to $10,000 for'each <br /> 'day that the report is delinquent. At this time, staff is not proposing to request issuance of Pities for reports past <br /> due. However, failure to submit the required the annual report due 30 November 1999, will result in enforcement <br /> action. <br /> Enclosed are two copies of a form titled "Annual Report For Dairies Subject To Monitoring And Reporting' <br /> Program No. 96-270-01." Please use one copy to file your annual report due 30 November 1999. Please retain <br /> the other copy as a blank original for your files and use it to make copies for reports due in subsequent years. It <br /> is your responsibility to file the reports each year, and failure to do so can result in enforcement action. <br /> You will note that WDR Order No. 96-270 requires preparation of a "Nutrient and Irrigation Water Management <br /> Plan and references Fact Sheet No. 4 for Dairies.
