CONDITIONS OF APPROVAL -3- <br /> BUFFALO TANK CORPORATION <br /> SA-93-0011 <br /> PUBLIC HEALTH SERVICES: <br /> 1. The PHS/EHD, Unit IV states that surface and subsurface <br /> contamination as a result of unauthorized releases from <br /> both above and below ground storage tanks exist. As of <br /> this date, no work plan to investigate the <br /> contamination has been submitted to the Site Mitigation <br /> Unit. Site is currently on the UST Contamination Site <br /> List. Immediately contact Diane Hinson, Site <br /> Mitigation Unit, (209) 468-3452 for assessment <br /> information. <br /> 2 . The well must be repaired under inspection by the <br /> Environmental Health Division (San Joaquin County <br /> Ordinance 1862, Section 5-4100 as follows: Seal <br /> domestic well (needs chlorinator plug and caulking) . <br /> 3 . Refuse, garbage and other solid waste materials must be <br /> stored, removed and transported to an approved landfill <br /> in accordance with San Joaquin County Ordinance 2887, <br /> Section 5-2400. <br /> 4 . Demolition of all dilapidated structure(s) and removal <br /> of all demolition material from premises to an approved <br /> landfill site in accordance with San Joaquin County <br /> Ordinance 2887, Section 5-2401. <br /> 5. If floor drains are required, they must be plumbed <br /> through a sand/oil separator and discharged to an above <br /> ground system approved by the Environmental Health <br /> Division. This system must be designed by a registered <br /> engineer. If the above ground holding facility had to <br /> be emptied you first would have to analyze the contents <br /> for any hazardous material. You can obtain specific <br /> test requirements from the Environmental Health <br /> Division. <br /> AIR POLLUTION CONTROL DISTRICT: <br /> 1. The applicant shall apply for an Authority to Construct <br /> and a Permit to Operate for the two approved fuel <br /> tanks. <br /> 2 . Submerged fill pipe controls shall be required for the <br /> fuel tanks. <br />
