The number of times the wastewater from the treatment -P ond was discharged to the cropland <br /> during the previous year. <br /> 8. The crop(s) grown on the cropland and yield during the previous year. <br /> 9. Descriptions of off-property discharges including: <br /> • Date and time of discharge; <br /> • Duration and location of discharge; <br /> • Source of waste discharged; and <br /> • Corrective actions(s) taken, including date of action(s). <br /> 10. Changes in the ownership or operator including the current name, address and telephone number. <br /> The results of any monitoring done more frequently than required at the locations specified in the <br /> Monitoring and Reporting Program shall be reported to the Board. <br /> In addition, the report shall contain a discussion of the compliance record and the corrective actions <br /> taken or planned that maybe needed to bring the discharge into full compliance with the waste <br /> discharge requirements. <br /> The Discharger shall implement the above monitoring and reporting program on the effective date of <br /> this Order. <br /> Ordered by: <br /> RON.
