MONITORING AND REPORTING PROGRAM ORDER NO. -3- RICHARD A. STAGNO, AND STAGNO'S MEAT COMPANY SAN JOAQUIN COUNTY from the monitoring program after the fifth year, provided that significant changes in water quality have not occurred. REPORTING In reporting the monitoring data, the Discharger shall arrange the data in tabular form so that the date, the compounds, and the concentrations are readily discernible. The data shall be summarized in such a manner to illustrate clearly the compliance with waste discharge requirements. Analytical data sheets must be included. The annual report shall be submitted to the Regional Board by the 31st of January of each year and shall cover all waste disposal activities during the previous year. The annual report shall also contain the following information: 1. Date of the report; name, address, and telephone number of report preparer. 2. The dates and results of all inspections and sampling events as required above. 3. The average daily population of animals at the facility during the 120 day period of maximum animal confinement. 4. Records of all off-site manure transactions during the previous year including volumes and destination. 5. Maintenance of freeboard. 6. Maintenance of liner integrity. 7. The number of times the wastewater from the treatment pond was discharged to the cropland during the previous year. 8. The crop(s) grown on the cropland and yield during the previous year. 9. Descriptions of off-property discharges including: • Date and time of discharge; • Duration and location of discharge; • Source of waste discharged; and • Corrective actions(s)taken, including date of action(s). 10. Changes in the ownership or operator including the current name, address and telephone number. The results of any monitoring done more frequently than required at the locations specified in the Monitoring and Reporting Program shall be reported to the Board. In addition, the report shall contain a discussion of the compliance record and the corrective actions taken or planned that may be needed to bring the discharge into full compliance with the waste discharge requirements.
