The <br /> work shall be allowed from 9:00 AM to 3:00 PM. A copy of this permit shall be placed in the <br /> vehicles involved in doing this work. <br /> In accordance with General Provision No. 6, SEVEN WORKING DAYS PRIOR to the start of <br /> this work, Permittee shall notify the State Representative JOSEPH A.VALDEZ,P.O. Box 2048 <br /> Stockton, CA 95201, (209) 948-3656. All work requiring traffic control must apply for a <br /> closure number days prior to start of work. No closures on HOLIDAYS or WEEKENDS. <br /> All work under this permit must meet with the approval of the State Representative. <br /> Failure on the Permittee's part to comply with any provision will be cause for revocation of this <br /> permit. <br /> IMMEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, <br /> PERMITTEE SHALL FILL OUT AND MAIL NOTICE OF COMPLETION PROVIDED BY <br /> GRANTOR. <br /> Permittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State <br /> Standard Specifications Reference to Engineer in the State Standard Specifications shall include <br /> State Representative. <br /> Permittee is directed to Standard Specification section 7-1.11 Preservation of Property, and <br /> Business and Professions Code, Section 8771. Permittee shall physically inspect the work site <br /> and locate survey monuments prior to work commencement. Monuments shall be referenced or <br /> reset in accordance with Business and Professions Code. <br /> Traffic control shall be done in conformance with Caltrans Manual on Uniform Traffic <br /> Control Devices (MUTCD) 2003. <br /> In addition to General Provision 35, "Notification of department and TMC", the Permittee <br /> shall notify the Department's Representative AND THE TRANSPORTATION <br /> MANAGEMENT CENTER (TMC) immediately prior to initiating any traffic control and <br /> immediately upon the removal of any traffic controls. The telephone number for Caltrans <br /> District 10 TMC is (209) 948-7556. <br /> Flashing arrow signs (Type I) are required for lane closures on expressways or multilane <br /> conventional highways. <br /> A pre-job conference with the Permittee, Contractor, and State Representative may be required <br /> prior to start of work. Permittee shall contact the State Representative and arrange the meeting. <br /> This meeting may be waived at the discretion of the State Representative. <br /> Notwithstanding General Provision #4, your contractor is required to apply for and obtain an <br /> encroachment permit prior to start of work. <br /> Permittee shall, prior to commencement of any work, provide the State Representative with the <br /> name and phone number of the person in responsible charge of the work to be performed <br /> under this permit. <br />
