060185650 <br /> , April 7,2006 <br /> Page 3 of 5 <br /> Failure on the Permittee's part to comply with any provision will be cause for revocation of this <br /> permit. <br /> IMMEDIATELY FOLLOWING COMPLETION : OF WORK PERMITTED HEREIN, <br /> PERMITTEE SHALL FILL OUT AND MAIL NOTICE OF COMPLDETION PROVEDED BY <br /> GRANTOR. <br /> Permittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State <br /> Standard Specifications Reference to Engineer in the State Standard Specifications shall include <br /> State Representative, <br /> Permittee is directed to Standard Specification section 7-1.11 Preservation of Property, and <br /> Business and Professions Code, Section 8 77 1. Permittee shall physically inspect-the work site <br /> and locate survey monuments prior to work commencement. Monuments shall be referenced or <br /> reset in accordance with Business and Professions Code. <br /> In accordance with Section 6500 of the Labor Code, a Cal-OSHA permit is required prior <br /> to the start of work. <br /> In addition to the attached General Provisions, the following. special provisions are also <br /> applicable for installation/abandonment of groundwater monitoring wells: <br /> 1. Unless specifically authorized elsewhere in this permit, work shall not be performed: <br /> A. On holidays or an afternoon proceeding a holiday. <br /> B. In medians. <br /> C. When work requires traffic control on lanes: . <br /> 2. Permittee shall furnish, place and maintain required signs, safety equipment, traffic control, <br /> and warning devices in accordance with the manual of Traffic Controls published by the <br /> Department of Transportation. <br /> 3. All personnel shall wear hard hats and orange vests, shirts or jackets as appropriate. <br /> 4. Unless specifically authorized in this permit, markings within the right of way shall be <br /> temporary. Paint used for markings shall be water-soluble and other markers shall be k <br /> removed upon completion of work. <br /> 5. Any collected data requested by Caltrans shall be furnished to Caltrans without charge. l <br /> 6. Vehicles necessary for work may be parked in State Highway right.of way off paved <br /> shoulders. Auxiliary support and employee vehicles shall remain outside the right of way. <br /> 7. Permittee shall provide for the safe passage of pedestrians.' <br /> . k <br />
