An Environmental Questionnaire and Disclosure Statement was sent to Mr.George Garcia. At the time this report was completed he had not finished the questionnaire and indicated we should proceed without it. lr 3.0 PAST AND CURRENT SOURCES OF CONTAMINATION L3.1 Research Methods NOA employs a standard practice for review of database information. On a street map showing the L location of the subject property,a circle is drawn one-quarter mile out from the property boundaries, and street maps or web-based mapping services are then used to determine address ranges for all streets V within the quarter mile. All agency databases searchable by address are then searched for these address L ranges. i AL2 Regional Water Quality Control Board L Our office reviewed the Leaking Underground Storage Tanks(LUST)list Quly 2004), the Water PunAyors Quarterly Report on MTBE Quly 2004), and the Spills, Leaks, Investigations, and Cleanups(SLIC)list (April Ir 2004)on file with the Central Valley Regional Water Quality Control Board(CVRWQCB)in an attempt to determine if there are any files present at that agency which note the unauthorized release of a toxic L or hazardous agent into the environment on the subject property or within a one-quarter mile radius of the subject property. The subject property was not listed on any of these databases,and no addresses within one-quarter mile of the subject property were found on the databases. L 3.1.2 San Joaquin County Environmental Health Department According to Ms. Kimberly Campbell of the San Joaquin County Environmental Health Department (EHD), no files were available for the subject property addresses of 5113 & 5153 East Main Street (personal communication, October 15,2004). The Undergmund Storage Tank Site Mitigation List, dated June 24, 2004, was reviewed at the EHD on October 11,2004,no sites within one-quarter mile of the subject property were found on this list The LOP/Non-LOP Sites list,dated July 20, 2004,was also reviewed at the EHD on October 11,2004;the following site within one-quarter mile was found: Charlie s Auto Parts&Dismantlers 4515 E.Main St. 4 L .4A L
