NOTE: The project proposes to keep one well for irrigation purposes. The Environmental Health <br /> Department cannot support the proposed location in the parking lot driveway without providing raised <br /> barrier posts for protection. The Environmental Health Department recommends that the parking lot <br /> be redesigned to align the well in a no-parking area, or destroy the well under permit and inspection. <br /> d. The applicant shall obtain written approval for the sewage treatment system from the California <br /> Regional Water Quality Control Board (Water Code, Article 5, Section 13280). <br /> e. Construction of a commercial engineered/alternative sewage disposal system under permit and <br /> inspection by the Environmental Health Department is required at the time of development <br /> (Development Title Sections 9-1110.3 and 9-1110.4). A construction permit fee of$890.00 is due at <br /> the time of submittal. <br /> f. The Environmental Health Department will require the packaged treatment system to be under an <br /> annual operating permit with the Environmental Health Department, which is currently a$450.00 <br /> annual operating fee (Development Title Section 9-1105.2) <br /> g. The Environmental Health Department will require a certified operator for the overall operation of the <br /> wastewater treatment plant. (Development Title Section 9-1110.5) <br /> h. State on revised plans the maximum number of parishioners and visitors the sewage disposal <br /> system is being designed for. In addition, show on revised plans that the leach field area will be <br /> barricaded so that it cannot be driven over, parked on, or used as a storage area. This leach field <br /> area must be used for that specific purpose only and cannot contain any underground utility lines. <br /> (Development Title Section 9-1110.4[c][51) <br /> i. Connect to an approved public water supply system as required by Development Title Section 9- <br /> 1120.2.
