. ADDIS UNAL INFORMATION REQUIREMENTS MUNN AND PERKINS,ESCALON, SAN JOAQUIN COUNTY Please provide the following information: 1: Provide site information which includes the following: a. Depict the site on a 7.5 minute U.S.G.S quadrangle map and indicate'the township,.range, and section of the area; b. Identify Assessors Parcel Number(s); c. Identify the area of the site, in acres, and the total acreage of parcel; d. Indicate uses of surrounding land (i.e., agricultural, residential, recreation, etc.) and the site; j e. Report annual average precipitation and evaporation; and 2. Provide a site mapthat shows property lines buildings, storage ponds, mining areas, abandoned tailing ponds, and surface water drainage courses within the_sit-e -and within 1,000 feet of the site. 3. Describe the surface water drainage patterns around the site. Describe the disposal of stormwater ' runoff from site including areas surrounding the mining operations, processing operations, product storage piles and waste piles. j 4. Provide a brief description of the geology and;hydrogeology of the area. If the site is located in an on-stream deposit in a watershed previously mined for gold,then a wastewater sample and a sediment sample (from the wastewater pond) shall be collected and analyzed for mercury. The detection limit for the wastewater analysis shall be at least 1.0 nanogram/liter. The wastewater sample shall be filtered prior to digestion and analysis. Please contact the Regional Board staff member for further clarification prior to performing any mercury sampling. 5. Are any areas of the site located within the 100 year flood zone? G. Do any active or abandoned industrial; agricultural, domestic, or groundwater wells exist on the property? If yes, provide a map showing their location(s). Include data on the depth of groundwater and information on aquifers and aquitards identified beneath the property. 7. Describe the area(s) to be excavated and the projected schedule for each phase. Include estimates of the amount of sand and gravel that will be produced each year and the volume of wastewater that will be produced. 8. Describe the products that are manufactured or are planned to be manufactured on-site (e.g. asphaltic concrete, concrete, aggregate, etc.). Describe the complete processing operation(s) from mining through the refining process including types of finished products. Provide a complete list all chemical additives used in the mining or refining process. For each chemical listed provide the Material Safety Data Sheet. Describe the process in which each chemical is used along with the daily dosage rate. If concrete wastewater is generated or disposed of at the site,please contact the Regional Board staff member for further clarification prior to submitting the RWD. 9. Describe the generation,transportation, and disposal of all wastewater including domestic wastewater and industrial wastewater. Describe any laboratory processes performed on-site. Of special concern is gold assay procedures and any gold recovery procedures such as amalgamation and/or cyanide leach processes.
