March 28,2005
NOA Project Number: E05007A
Product Name Active Ingredient 00 01 02 03 04
L
Dimethoate 4EC dimethoate L
Kelthane MP dicofol X
MCPA Amine MCPA Amine
Plant Safflower unknown
The EPA's Restricted Use Products (RUP)Report10 was searched for the chemicals reportedly applied
to the subject property. Based on this source,it appears that, of the agricultural chemicals reportedly
` applied to the subject property, only Lannate 90SP Insecticide (containing methomyl) is an RUP. The
U.C. Davis Extension Toxicology Network (EXTOXNET) web site" was consulted for additional
information on this chemical.
According to EXTOXNET,methomyl is a broad spectrum insecticide classified as an RUP by the EPA
because of its high acute toxicity to humans. Methomyl is highly soluble in water, has a soil half-life of
` 33 days, and its adsorption to soil particles is weak to moderate. Consequently, methomyl has been
identified as a potential groundwater contaminant (Pesticide Information Profile, 1993).
3.L5 [dater Supply and Sewage Disposal System
No wells or septic systems are located on the sub'ect prov=. The EHD well and septic permit files
were sercM on Marchiii an effort to obtain copies of permits for any water wells or septic
tanks on the subject property;no permits were found.
3.L6 Department of Oil, Gas, and Geothermal Resources
The California Department of Oil,Gas,and Geothermal Resources(DOGGR)Wildcat Map 60612 was
` reviewed for evidence of oil or gas wells on or in the vicinity of the subject property. No wells are
depicted within one-quarter mile of the subject property.
3.L7 Site Reconnaissance
A visual reconnaissance of the subject property was conducted on March 22,2005. Photographs of the
b' subject property are attached to this report as Appendix B. A site visit/assessment checklist is attached
to this report as Appendix D.
'()U.S.Environmental Protection Agency(EPA),June 2003,Restricted Use Products(RUP)Report.
"University of California,Davis,2004,Extension Toxicology Network(EXTOXNET),
http://extoxnet.orst.edu/ghindex.html.
6. 12 California Department of Oil,Gas,and Geothermal Resources,2005,Wildcat Map 606.
