MONITORING AND REPORTING PROGRAM _g <br /> CITY OF LODI <br /> WHITE SLOUGH WATER POLLUTION CONTROL PLANT <br /> SAN JOAQUIN COUNTY <br /> The results of any monitoring done more frequently than required at the locations <br /> specified in the Monitoring and Reporting Program shall be reported to the Board. <br /> Upon written request of the Board, the Discharger shall submit a report to the <br /> Board by 30 January of each year. The report shall contain both tabular and <br /> graphical summaries of the monitoring data obtained during the previous year. <br /> In addition, the Discharger shall discuss the compliance record and the <br /> corrective actions taken or planned which may be needed to bring the discharge <br /> into full compliance with the waste discharge requirements. <br /> ANNUAL LAND MANAGEMENT REPORTS <br /> The Discharger shall submit a summary report on the land management operation <br /> after the conclusion of each season. The report shall discuss total water <br /> application over the season; the total volume of reclaimed water( both domestic <br /> and industrial ) and sludge applied; the total nutrient loading from wastewater, <br /> sludges, and chemical fertilizers; and the amount of nutrients removed through <br /> harvest of the crop. In short, the report shall present a mass balance relative <br /> to pollutants of concern and hydraulic loading. The report is due by 30 January <br /> of the following year. <br /> PRETREATMENT PROGRAM REPORTS <br /> The Discharger shall submit annually a report to the Board, with copies to the <br /> EPA Regional Administrator and the State Board, describing the Discharger's <br /> pretreatment activities over the previous 12 months. In the event that the <br /> Discharger is not in compliance with any conditions or requirements of this <br /> Order, the Discharger shall include the reasons for noncompliance and state how <br /> and when the Discharger shall comply with such conditions and requirements. This <br /> annual report is due by 28 February and shall contain, but not be limited to, <br /> items G. 1 through G. 10 of Standard Provisions. <br /> In addition to the information required in the annual report, the Discharger <br /> shall report quarterly the information contained in G.4 (a throughg) of Standard <br /> Provisions. The reports shall also describe progress towards compliance with <br /> audit or pretreatment compliance inspection requirements. Reports shall be <br /> submitted within 30 days of the end of each quarter, excepting information <br /> required in the fourth quarterly report shall be included as part of the annual <br /> report. If none of the aforementioned conditions exist, at a minimum, a letter <br /> certifying that all industries are in compliance and no violations or changes to <br /> the pretreatment program have occurred during the quarter must be submitted. <br />
