Notifications in the Event of an Emergency <br /> or Permit Violation <br /> This notification process should be immediately followed in the <br /> event neither Del Kerlin, T. Brett Moroz, nor Mike Schafer is <br /> available. (Refer to the White Slough Operating Log for home <br /> phone numbers) <br /> Fran Forkas and Richard Prima <br /> California State CES — 1-800-852-7550 <br /> Regional Board - Robert Fagerness — (916) 255-3022 <br /> S.J. County OES — Phillip Cook — 468-3969 <br /> National Response Center — 1-800-424-8802 <br /> The State OES will notify Fish and Game. <br /> For Any Permit Violation to Receiving Waters <br /> Note time and effluent flow, take samples of the effluent and <br /> receiving water at the discharge point. Testing should be run <br /> immediately for samples, which can't be preserved. Visually <br /> observe the discharge point and the surrounding area. Record <br /> all observations. The above information should be available <br /> before notification is made. <br /> For each of the calls above to an outside authority, make note of the <br /> time and name of person you spoke to.
