Reporting of Results The permittee shall submit short <br /> and accurate quarterly reports to the Director containing <br /> the following informati=on; <br /> (a) Results of the injection fluid analyses specified in <br /> permit 'condition Part II, Section D, Item 3 . <br /> (b) Average, maximum, -and minimum monthly values for the <br /> continuously monitored parameters specified in Part <br /> II Section D, Item 4 . <br /> (c) Equipment calibration. <br /> (d) A narrative description of all non-compliance that <br /> occurred during -the reporting period. <br /> Monitoring results obtained during each calendar month <br /> shall be summarized for each month and reported on EPA <br /> Form 7520-8 .
