Infection Volume (Rate) Limitation: <br /> The maximum injection rate shall be 200 gpm. Injection is <br /> also limited to a rate that will not cause the injection <br /> pressure at the wellhead to exceed the limits subsequent to <br /> submission of a valid step rate test data discussed above. <br /> Injection Well Monitoring Program: <br /> The permittee is required to sample and analyze the water <br /> quality of the injected fluids at monthly intervals. The <br /> parameters analyzed are detailed in the permit. Similar <br /> analyses shall be conducted whenever the source of the <br /> injection fluid ch-anges. Monitoring of the injection <br /> pressure, annulus pressure, and injection flow rate must be <br /> done continuously. <br /> Section D - Plugging and Abandonment <br /> EPA has reviewed and approved the plugging and abandonment <br /> plan submitted by the applicant. The plugging and abandonment <br /> plan is incorporated into the permit as Attachment A. The <br /> estimated cost to plug and abandon NCPA' s STIG #1 well is <br /> $18, 300. <br /> Section E - Financial Responsibility <br /> The permittee has a bond rating within the four highest <br /> categories of Standard and Poor's (AAA, AA, A, or BB) or <br /> Moody' s (Aaa, Aa, A, or Baa) . EPA has identified NCPA as a <br /> financially healthy company and requires a financial credit <br /> rating statement to demonstrate financial responsibility. <br /> However, if NCPA fails the financial test, it must submit to <br /> EPA an alternative financial instrument to cover the cost of <br /> plugging and abandoning the NCPA STIG #1 well. <br /> 7 <br />
