1 Monitoring Devices: <br /> The operator is required to install pressure gauges designed <br /> to provide (1) a full pressure range of 100 percent greater <br /> than the anticipated operating pressure, and (2) a certified <br /> deviation accuracy of five (5) percent or less on the tubing <br /> and the tubing/casing annulus of the well. <br /> A flow meter will -be installed for measuring flow rates and <br /> cumulative volumes.* -- The meter will be certified for at least <br /> 95 percent accuracy throughout the range of injection rates <br /> used. <br /> A sampling tap will be installed on the injection pump <br /> discharge line for the purpose of periodically obtaining <br /> representative samples of the injection fluid. <br /> Section D - Corrective Action <br /> Three deep wells have been previously drilled within one mile <br /> of the STIG #1 location. These wells penetrated the proposed <br /> injection zone. Records filed with California Division of Oil <br /> and Gas (CDOG) show that these three wells have been abandoned <br /> following CDOG -procedures, and that the plugging operations <br /> were approved in every case by CDOG. Consequently, no further <br /> corrective action plan is necessary. <br /> No corrective action for wells located within the Area of <br /> Review will be required pursuant to 40 CFR 144 . 55 and 40 CFR <br /> 146. 7 . <br /> Section E - Well Operation <br /> Mechanical Integrity: <br /> Demonstrations of mechanical integrity of the injection <br /> casing, tubing and packer will be conducted within 30 days <br /> after any workovers or alterations and prior to recommencing <br /> injection. Continuation of injection operations may continue <br /> if the permittee demonstrates_ that the well has mechanical <br /> integrity in accordance with 40 CFR 146. 8 and has received <br /> written notice from the Director that such a demonstration is <br /> satisfactory. The mechanical integrity tests (MIT) will be <br /> performed in accordance with requirements set forth by the <br /> Director. <br /> An MIT pressure test of the annulus will also be conducted at <br /> least once every year during the life of the permit. <br /> The construction details and cement records satisfy the <br /> requirements of 40 CFR 146. 8 (a) (2) for demonstrating the <br /> absence of significant fluid movement. <br /> 6 <br />
