l
2 . Tubing and Packer Specifications Injection will take
place only through the 4 1/2 inch tubing with a Baker
Lok-Set Packer set within the long string casing at
approximately 892 feet above the injection zone. The
tubing and packers shall be maintained to prevent the
movement of fluids into an underground source of drinking
water. The tubing and packers shall be corrosion-
resistant and of sufficient strength to resist--wellbore
pressures developed during the life of the well:. The
casing/tubing annulus will be filled with corrosion
inhibited fluid. Injection between the outermost casing
and the wellbore is prohibited.
3 . In-iection Intervals Injection into the intended zone is
through perforations made through the steel casing and
cement into the zone of injection by 11-gram jet charges
at 4 shots per foot, 120 degrees phasing, in 3-1/8 inch
hollow steel carrier guns. Perforations are 0. 31 inch
diameter holes per foot penetrating 18 .9 inches.
Injection shall be permitted for the lower Domengine
Formation, between 4234 feet and 4507 feet below the
ground- surface. Alteration of the injection perforations
and other rework operations must be properly reported
(EPA Form 7520-12) and the well must demonstrate
mechanical integrity before injection is authorized.
4. Monitoring Devices The operator shall install and
maintain in good operating condition:
(a) A tap on the suction line prior to the injection
pump and the wellhead for the purpose of obtaining
representative samples of the injection fluids;
(b) A flow meter with measured cumulative volumes that
are certified for a deviation accuracy of five (5)
percent or less throughout the range of injection
rates allowed by the permit; and
(c) A pressure gauge designed to provide (1) a full
pressure range of 100 percent greater than the
anticipated operating pressure, and (2) a certified
deviation accuracy of five (5) percent or less.
5. Proposed Changes and Workovers The permittee shall give .
advance notice to the Director, as soon as possible, of
any planned physical alterations or additions to the
permitted injection well. Any changes in the well
construction will require prior approval of the EPA and a
permit modification under the requirements of 40 CFR
Page 6 of 29
UIC Permit #CA194000002
