Tubing and Packer Specifications Injection will take <br /> place only through the 4 1/2 inch tubing with a Baker <br /> Lok-Set Packer set within the long string casing at <br /> approximately 892 feet above the injection zone. The <br /> tubing and packers shall be maintained to prevent the <br /> movement of fluids into an underground source of drinking <br /> water. The tubing and packers shall be corrosion- <br /> resistant and of sufficient strength to resist--wellbore <br /> pressures developed during the life of the well:. The <br /> casing/tubing annulus will be filled with corrosion <br /> inhibited fluid. Injection between the outermost casing <br /> and the wellbore is prohibited. <br /> 3 . In-iection Intervals Injection into the intended zone is <br /> through perforations made through the steel casing and <br /> cement into the zone of injection by 11-gram jet charges <br /> at 4 shots per foot, 120 degrees phasing, in 3-1/8 inch <br /> hollow steel carrier guns. Perforations are 0. 31 inch <br /> diameter holes per foot penetrating 18 .9 inches. <br /> Injection shall be permitted for the lower Domengine <br /> Formation, between 4234 feet and 4507 feet below the <br /> ground- surface. Alteration of the injection perforations <br /> and other rework operations must be properly reported <br /> (EPA Form 7520-12) and the well must demonstrate <br /> mechanical integrity before injection is authorized. <br /> 4. Monitoring Devices The operator shall install and <br /> maintain in good operating condition: <br /> (a) A tap on the suction line prior to the injection <br /> pump and the wellhead for the purpose of obtaining <br /> representative samples of the injection fluids; <br /> (b) A flow meter with measured cumulative volumes that <br /> are certified for a deviation accuracy of five (5) <br /> percent or less throughout the range of injection <br /> rates allowed by the permit; and <br /> (c) A pressure gauge designed to provide (1) a full <br /> pressure range of 100 percent greater than the <br /> anticipated operating pressure, and (2) a certified <br /> deviation accuracy of five (5) percent or less. <br /> 5. Proposed Changes and Workovers The permittee shall give . <br /> advance notice to the Director, as soon as possible, of <br /> any planned physical alterations or additions to the <br /> permitted injection well.
