Box 774 <br /> Lodi, CA 95241 <br /> Dear Sirs: <br /> INFORMATION REQUEST FOR AG FLIGHT - 6015 WEST ACAMPO, LODI, <br /> CALIFORNIA 95241 <br /> The California Department of Health Services (Department) has <br /> received information that the above mentioned business has stored <br /> and/or handled hazardous materials (as defined in the Attachment) <br /> at the Lodi Airport, 23987 North Highway 99 , Acampo, CA. <br /> Analysis results of samples taken from Lodi Airport by the <br /> Department revealed soil contamination with DDT, DDE, 2 , 4-D, <br /> toxaphene and other hazardous substances. <br /> As part of the investigation, the Department is requesting <br /> historical information from you as a past operator at the Lodi <br /> Airport. Under Section 25358 . 1 of the California Health and <br /> Safety Code (H&SC) , any duly designated officer, employee, or <br /> representative of the State may require persons subject to <br /> Chapter 6. 8 of H&SC to furnish information necessary for the <br /> Department to determine the appropriate response to a release of <br /> hazardous substances or wastes. <br /> Therefore, the Department is requesting the information described <br /> in the Attachment to this letter pursuant to Chapter 6. 8 of the <br /> California H&SC. <br /> Compliance with the site specific request for information set <br /> forth in the Attachment is mandatory. Failure to respond fully <br /> and truthfully to this request within thirty (30) days of receipt <br /> of this letter, or to adequately justify such failure to respond, <br /> may result in enforcement action (enforcement action may include, <br /> but is not limited by civil penalties including fines of up to <br /> $25, 000 per day per violation) by the Department, pursuant to <br /> Chapter 6. 8 of the California H&SC, Sections 25358 . 1 and 25367 . <br />
