No person may treat hazardous waste onsite without submitting notification sixty (60) days prior to <br />beginning the first treatment of hazardous waste. Each notification submitted shall include the following information.. <br />-The name, identification number, site address, mailing address, and telephone number of the generator to whom <br />the CA is granted. <br />-A description of the physical characteristics and chemical composition of the hazardous waste to which the CA <br />applies. <br />-A description of the hazardous waste treatment activity to which the CA applies, including the basis for determining <br />that a hazardous waste facilities permit is not required under the federal act. <br />-A description of the characteristics and management of any treatment residuals. <br />-Documentation of any convictions, judgments, settlements, or orders resulting from an action by any local, state, or <br />federal environmental or public health enforcement agency concerning the operation of the facility within the last <br />three years. <br />Immediately submit the required hazardous waste treatment notification on CERS (cers.calepa.ca.gov). <br />This is a Class II violation. <br />104 <br />CCR 67450.13(a)(1) Failed to submit an estimate of closure costs. <br />An estimate of closure costs was not submitted in CERS. All closure cost estimates must be accompanied by a <br />written Estimate of Closure Costs. The fixed treatment unit (FTU) owner or operator shall prepare a written <br />estimate of the cost of closing each unit. The estimate shall equal the actual cost or the costs estimated that would <br />be incurred for closing a treatment unit when using the owner's own staff and/or personal equipment. The closure <br />cost estimate may take into account any salvage value that may be realized from the sale of wastes, facility <br />structure or equipment, land or other facility assets. This estimate shall be submitted as an attachment to the <br />Financial Assurance Certification. Immediately upload an Estimate of Closure Costs to the CERS website <br />(cers.calepa.ca.gov). <br />This is a Class II violation. <br />105 <br />CCR 67450.13(a)(2) Failed to adjust the closure cost estimate by March 1. <br />An adjustment for the closure cost estimate was not completed by March 1 of each year. The most recent closure <br />plan submitted was dated March 2014. The Owner/Operator or Generator shall adjust the closure cost estimate for <br />inflation by March 1 of each year.
