Proposal to Prepare SPCC and Hazardous Waste Contingency Plass <br />Covey Auto Express Inc. <br />1444 EI Pinal Drive <br />Stockton, Califomia <br />Page 2 <br />o Discharge prevention and control measures <br />o Discharge countermeasures and recovered material disposal methods <br />o Facility contacts <br />• Discharge notification information and procedures <br />• Potential spill predictions (volumes, rates, and quantities) <br />• Written inspection and/or test procedures <br />• Personnel training[ <br />• Site security <br />Condor will also include a proposed checklist for monthly inspections, any identified spill history, site <br />photos, a proposed training documentation form, and a Certification of Applicability of Substantial Harm, <br />Covey Auto Express Inc. will be required to provide Condor with the following: Facility contact <br />information; a site map (an AutoCAD version, if available); description of the Facility and operations; <br />copy of current emergency response/spill response plan; manufacturer's information on oil tanks, fill and <br />delivery systems, and any other oil storage devices; any tank testing and inspection records; and a list of <br />contractors that provide spill clean-up services. <br />Following completion of the site visit, a Hazardous Waste Contingency Plan will be prepared in <br />accordance with Title 22 of the California Code of Regulations, including the following components: <br />1. Emergency Procedure to be initiated by Emergency Coordinator including Spill Notification <br />2. A Coordination with Emergency Responses agencies <br />3. List of Emergency Coordinators <br />4. List of Emergency Equipment and required Maintenance/Testing <br />5. Evacuation Plan <br />6. Current phone of the California Emergency Management Agency <br />7. A written Employee Training Program, including documentation. <br />SCHEDULE AND COST ESTIMATE <br />We are prepared to begin upon verbal authorization to proceed and receipt of a signed work order <br />(attached). We anticipate providing a draft copy of the SPCC and Hazardous Waste Contingency Plans <br />for your review within three to four weeks following the Facility inspection. If this time frame does not <br />meet your current needs, please call us to discuss your schedule. Following your review, the SPCC and <br />Hazardous Waste Contingency Plans will be finalized within one to two weeks. <br />Our estimated cost for the proposed scope of work is $4,600.00. A detailed breakdown of the estimated <br />cost is provided in the attached Cost Estimate dated July 30, 2010. Condor will perform the work <br />described herein on a time -and -expense basis in accordance with the attached Master Services Agreement <br />for Consulting Services (MSA) and Work Order Agreement dated July 30, 2010. Work will be charged in <br />accordance with Condor's current Schedule of Fees (attached).
