Prepare a plan as specified in the <br />regulations. Use the regulations copy provided by the inspector as a guide. Train the site employees <br />annually on the HW contingency plan. Send a copy of your HW contingency plan to the Environmental <br />Health Department (EHD) by August 2, 2010. <br />60,63) Failed to keep HW disposal records for three years. The facility generates waste absorbent, oil- <br />4Vat—separator waste (northwest corner of the office building) and coolant (antifreeze) waste. The <br />facility did not have 2010, 2009 and 2008 disposal receipts for these wastes. Coolant disposal receipts <br />for 4-26-10, 12-22-09, 11-23-09 were reviewed. Send copies of these disposal manifests or receipts to <br />the Environmental Health Department (EHD) by August 2, 2010. <br />Summary of Violations: 8,11,20,23,27,32,35,44,49,60,63 - all minor. <br />Notes: <br />1) Fill out and return the following by August 2, 2010: Return to Compliance Certification form, a written <br />statement explaining how each violation was resolved and supporting documentation. <br />2) The facility generated 40 tons of hazardous waste in 2009. The hazardous waste generator <br />operating permit will be changed to a 25-50 tons/year permit. <br />3) The facility generates waste oil, coolant, oil filters, parts washer waste, used absorbent, and oil -water <br />separator waste.
