The <br />designated operator indicated NA (not applicable) for number 4 (Has each alarm for the previous month been <br />responded to appropriately?) on the monthly inspection report. The missing alarms include: T1 regular unleaded <br />sudden loss alarm on 7/11/16, probe out alarms on 7/10 and 11/ 2016, and hi water warning alarm on 7/8 and <br />11/2016; and T3: unleaded sudden loss alarm on 7/11/16 and probe out on 7/10 and 11/ 2016. The alarms from the <br />last monitoring system certification (MSC) testing on 9/29/15 were not documented on the monthly print outs for <br />liquid sensors L9 and L10 found in the designated operator's monthly inspection reports beginning on 10/16/15 <br />through 7/22/16. Also the designated operator's monthly report for 6/19/16 has the panel print-out dated 6/10/16. <br />Also the designated operator failed to answer Item 4 on the 9/29/16 monthly inspection report. During the monthly <br />inspection, the designated operator shall review the alarm history for the previous month, check that each alarm <br />was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken <br />in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections <br />at this facility are including all of the required information on the reports. <br />The designated operator failed to visually inspect the under dispenser containments during the 9/29/16 monthly <br />inspection. The designated operator failed to see that the 208 sensor had fallen over in dispenser 7/8. During the <br />monthly inspection, the designated operator shall visually inspect all spill containers and UDC sumps, and any tank <br />top sump that had an alarm for which there is no record of a service visit. Ensure that designated operators <br />performing monthly inspections at this facility are inspecting all of the required areas and noting it on the on the <br />reports. <br />The designated operator failed to check and note the correct test dates for the monitoring system certification <br />(MSC), spill container testing, and secondary containment testing on the January 14, 2016 designated operator <br />monthly inspection report. Instead of entering 9/29/15 for the MSC and spill bucket testing, the date 2/24/15 was <br />entered; and instead of entering 9/26/14 for the seconary containment testing, 10/7/14 was entered. During the <br />monthly inspection, the designated operator shall check that all required testing and maintenance for the UST <br />system have been completed and document the dates they were done. Ensure that designated operators <br />performing monthly inspections at this facility are including all of the required information on the reports. <br />This is a repeat violation, Class II. <br />306 <br />CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br />Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. The 91 -product and <br />87 -product middle STP sump 208 sensors were located on the wrong side of the sumps and not located to detect a <br />leak at the earliest opportunity. Also the 91 -product and 87 -product east (nearest West Lane) STP 208 sensors <br />were leaning over and not in a vertical position; and the 208 sensor in under dispenser containment 7/8 had fallen <br />over. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity.
