STOCKTON September 28, 2016 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS Il,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the February 2016 <br /> through March 2016 designated operator monthly inspection report and failed to check that they were responded to <br /> appropriately. The missing alarms include: <br /> -T1:87 Overfill on February 25, 2016 <br /> -T1:87 Overfill on March 10, 2016 <br /> -T1:87 Overfill on March 24, 2016 <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 309 CCR 2636(f)(1) Monitoring in sump/ UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The 87 annular sensor failed to activate an audible and visual alarm when tested to ensure continuous monitoring of <br /> the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the <br /> flow of product at the dispenser when it detects a leak. Correct immediately by having a properly licensed, trained, <br /> and certified contractor replace the failed component with a functional component(LG 113-x listed, if applicable) <br /> and obtain a permit within one business day from the EHD. <br /> This is a minor violation. <br /> 320 CCR 2635(b)(2) Overfill prevention system not present or failed to meet overfill requirements. <br /> The audio/visual alarm used as the overfill prevention system is not currently functional on the 87, 91 and diesel <br /> UST system. Immediately discontinue deposition of petroleum into this tank until the component is repaired or <br /> replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. If the <br /> failed component can't be replaced immediately, there is a possibility that the 87, 91 and diesel UST system may be <br /> red tagged to prevent fuel inputs. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 28, 2016 . <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
