The following is an itemized list of underground storage tank violations that have not <br /> been addressed for FOOD MART GASOLINE` as of April 23, 2018. <br /> Open violations from September 29, 2017 inspection <br /> Violation #101 -No permit to operate. <br /> A permit to operate the UST system has not been issued due to an out standing violation that has not yet been <br /> corrected from 2016. No person may own or operate an UST unless a permit for its operation has been issued by <br /> the local agency to the owner or operator of the UST system. Immediately obtain a permit to operate a UST system <br /> from the EHD. <br /> Violation #121 - Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 9/28/2016 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by 10/28/2016. This <br /> information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br /> inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br /> actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br /> corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. <br /> The violation that was not corrected is the following: <br /> The designated operator did not inspect all of the required items. <br /> Violation #201 - Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. The facility and the designated <br /> operator failed to document daily alarms from the Veeder Root panel and failed to check that the alarms were <br /> responded to appropriately. <br /> On 10/13/2016, the following alarms were not documented: a probe out alarm for Tank2 91 unleaded, a probe out <br /> alarm for Tank Diesel and a L3 annular space fuel alarm. <br /> On 3/9/2017 and on 3/13/2017, a Tank Diesel probe out alarm occurred. <br /> On 4/5/2017, a Tank Diesel probe out alarm occurred. <br /> On 5/10/2017, 5/16/2017 and 5/19/2017, Tank 1 87 unleaded overfill alarms occurred. <br /> The designated operator(DO) report was completed early in the month of August 2017 and had not been completed <br /> for the month of September 2017. The following alarms occurred after the August DO report but was not reviewed <br /> by the facility. <br /> On 8/8/2017, a T3 Diesel probe out alarm occurred. <br /> On 8/28/2017, a Tank 1 87 unleaded overfill alarm occurred. <br /> Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration <br /> and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in <br /> the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test <br /> results; (6) results of inventory readings and reconciliations. <br /> Send in information to EHD describing how the alarm was responded to. <br /> Page 1 of 2 <br />
