The following is an itemized list of underground storage tank violations that have not <br /> been addressed for FOOD MART GASOLINE` as of April 23, 2018. <br /> Open violations from September 29, 2017 inspection <br /> Violation#209 - Designated operator did not inspect all required items and/or the inspection reports not <br /> completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the November 2016, <br /> March 2017, April 2017 and June 2017 designated operator monthly inspection reports and failed to check that they <br /> were responded to appropriately. The missing alarms include: <br /> On 10/13/2016, the following alarms were not documented: a probe out alarm for Tank2 91 unleaded, a probe out <br /> alarm for Tank Diesel and a L3 annular space fuel alarm. <br /> On 3/9/2017 and on 3/13/2017, a Tank Diesel probe out alarm occurred. <br /> On 4/5/2017, a Tank Diesel probe out alarm occurred. <br /> On 5/10/2017, 5/16/2017 and 5/19/2017, Tank 1 87 unleaded overfill alarms occurred. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> Violation#320 - Overfill prevention system not present or failed to meet overfill requirements. <br /> The annunciator used as part of the overfill prevention system is not currently functional on the 87 grade, 91 grade <br /> and Diesel underground storage tanks. Immediately discontinue deposition of petroleum into these tanks until the <br /> component is repaired or replaced by a properly licensed, trained, and certified contractor under permit and <br /> inspection of the EHD. If the failed component can't be replaced immediately, there is a possibility that the 87 <br /> grade, 91 grade and Diesel UST systems may be red tagged to prevent fuel inputs. <br /> Open violations from September 28, 2016 inspection <br /> Violation#204-Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> Violation#209 - Designated operator did not inspect all required items and/or the inspection reports not <br /> completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the February 2016 <br /> through March 2016 designated operator monthly inspection report and failed to check that they were responded to <br /> appropriately. The missing alarms include: <br /> -T1:87 Overfill on February 25, 2016 <br /> -T1:87 Overfill on March 10, 2016 <br /> -T1:87 Overfill on March 24, 2016 <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> Page 2 of 2 <br />
