PHS/EHD was not notified of the November 8, 1991 sampling event. Please be informed that
PHS/EHD inspections of sampling events are an important aspect of local regulatory oversight. Prior
to any site activity, including sampling, please contact Mary Meays at the number below 48 hours in
advance.
2. Earth Systems Environmental representative observed,November 8,1991,that MW2(total depth 60')
and MW3 (total depth 59') had gone dry and that MWl (total depth 65') contained free product.
The last recorded groundwater gradient (Weiss Associates Report, October 16, 1987) was to the
north-northeast. There may not be a downgradient monitoring well defining the extent of
groundwater contamination. A minimum of two additional monitoring wells will be required to
confirm the groundwater gradient, and to define the extent of soil and groundwater contamination.
Please submit a work plan for the additional investigation by February 14,1992. Additionally include
an interim remedial action plan to remove the free product from MW1. Please submit documentation
for the disposal of monitoring well development water and sampling purged water generated in 1987.
Reports, work plans and letters are to be submitted to both PHS/EHD and the Central Valley Regional Water
Quality Control Board (CVRWQCB) until the investigation and cleanup is adequate for closure. This will
facilitate concurrent and expeditious evaluation by both agencies.
If you have any comments, questions or to schedule a site inspection please contact Mary Meays,EHS,of my
staff at (209) 468-3441.
Jogi Khanna, M.D., M.P.H.
'ealth Oit,
r
rie A, ulla, REHS, ogram Manager
Environmental Health Division
LAC/MM:Ib
c: Elizabeth Thayer, CVRWQCB
c: James De Mera, Attorney for Mary Lehr
c: Jeff Meyer, Earth Systems Environmental
A Division of San]oaquin County Health Care Service
