Appendix A <br /> Quality Assurance/Quality Control Plan <br /> other forms, and in photographs. In addition, any deviations from the Workplan will be <br /> documented and justified accordingly. The Field Geologist/Scientist/Engineer has the authority to: <br /> • Stop any sampling that is not following acceptable protocols. <br /> • Discard any sample that is not correctly collected,preserved,or transported. <br /> • Invalidate any data obtained from an improperly collected sample or if the proper <br /> sample handling and documentation procedures have not been followed. <br /> A.2 FIELD QAIQC PROCEDURES <br /> Field QA/QC procedures implemented by the Field Geologist/Scientist/Engineer will include <br /> documentation of sample collection, sample handling, and field activities as well as the collection of <br /> field QC samples. These activities will be recorded on boring log forms, daily field logs, chain-of- <br /> custody forms and by photograph. The following field procedures will be implemented during the <br /> soil investigation to maintain sample quality control and integrity: <br /> • The photoionization detector (PID) will be calibrated daily in accordance with <br /> manufacturer specifications and documented on daily calibration logs; <br /> • All field procedures will be documented in daily field logs and by photograph; <br /> • Soil sample integrity will be maintained by labeling sample containers immediately <br /> following sample collection with a waterproof marker; <br /> • Sample collection will be documented on a chain-of-custody form. The chain-of- <br /> custody form will be filled out by the field sampling team at the time a sample is <br /> collected. The chain-of-custody will accompany the sample through all <br /> transportation until it is received by the laboratory, where it is filed. Whenever a <br /> sample is transferred from one party to another, both parties sign and date the <br /> chain-of-custody at the time of transfer; <br /> • All samples will be transferred from the field to a laboratory courier following chain- <br /> of-custody procedures;and, <br /> • Decontamination of drilling and sampling equipment will be conducted using a <br /> three-bucket wash system consisting of a detergent wash followed by one rinse with <br /> potable water and a final rinse with distilled water and/or using a steam cleaner unit. <br /> A.2.1 Calibration Procedures and Frequency <br /> All equipment and instruments used in the investigation will be maintained and calibrated to operate <br /> within the manufacturer's specifications so that the required sensitivity and QA/QC parameters are <br /> upheld. Operational procedures for all field equipment will be followed to ensure that the <br /> equipment is operating properly and that data are valid and traceable to a particular instrument. <br /> These procedures may include functional operational checks, routine maintenance, instrument <br /> TERRA PACIFIC GROUP Page A-2 of A-5 <br />
