Per your discussions with our Field Engineer <br /> Mike Thorsen, this letter requests a permit for the well being installed at the above address. The <br /> property is located adjacent to the alley running north from East Pine Street and is owned by the <br /> Buddhist Church of Lodi at 23 North Stockton Avenue, Lodi, California 95242. A copy of the <br /> access agreement is attached. <br /> The air sparge observation well is being moved from its proposed location a few feet to the east to <br /> avoid subsurface utility conflicts (see attached details). The work plan for the investigation, dated <br /> September 8, 2004, was approved by the Regional Water Quality Control Board (RWQCB)by letter <br /> dated October 13, 2004. Copies of the work plan and RWQCB approval letter were provided to <br /> your office as attachments to the November 11, 2004 application letter, which permitted wells <br /> proposed for City of Lodi public right of way. <br /> The same drilling contractor is being used and they are Layne Christensen Company of Fontana, <br /> California(C57 License No. 510011) and their Licensed Contractors Declaration (LCD)has been <br /> provided to your office previously. They were the driller for several of the previous phases of <br /> characterization. Underground Service Alert (U.S.A.) has been contacted to field locate <br /> underground utilities prior to commencing the field investigation. The diameter of the well borings <br /> will be eight(8) inches with a maximum depth of 70 feet. The well borings will be advanced using a <br /> drill rig equipped with hollow-stem auger equipment.
