Environmental Engineering, Consulting, and Remediation, Inc., <br /> dba E2C Remediation (E2CR), has prepared this well permit application and provides the requested <br /> information and permit fees for installation of remediation wells on private property. The wells <br /> located on City of Lodi public right-of-ways have already been permitted per E2CR's November 11, <br /> 2004 permit application letter. Per our discussions, this letter requests a permit for the four wells <br /> being installed on private property. The property (2 North Main Street, Lodi, California) is located <br /> at the northeast corner of North Main and East Pine Streets and is owned by Mr. Lal Atwal who lives <br /> at 300 Westwood Avenue, Lodi, California 95242. <br /> The four additional proposed wells at 2 North Main Street, Lodi, California (APN 043-07-112) are <br /> planned for the area east of the existing building in the area presently covered by a concrete slab. <br /> These wells include one air sparge well, two air sparge observation wells, and one SVE observation <br /> well (see attached details). The work plan for the investigation, dated September 8, 2004, was <br /> approved by the Regional Water Quality Control Board (RWQCB)by letter dated October 13, 2004. <br /> Copies of the work plan and RWQCB approval letter were provided to your office as attachments to <br /> the November 11, 2004 application letter, which permitted wells proposed for City of Lodi public <br /> right of way. If not too inconvenient, could you please also fax me a copy of the previously issued <br /> permits for this job or at least the permits for this latest phase. <br /> The drilling contractor will be Layne Christensen Company of Fontana, California(C57 License No. <br /> 510011) and their Licensed Contractors Declaration (LCD) has been provided to your office <br /> previously.
