Proiect Number 1804BK01 November 11 2004 <br /> wells will be constructed with 2-inch-diameter PVC casing and backfill materials as <br /> described in the attached Workplan. The annulus of each well above the sand pack will be <br /> backfilled to the ground surface using neat cement with approximately 5 percent bentonite <br /> powder in accordance with the requirements of the San Joaquin County Environmental <br /> Health Department well installation permit. The wells will be completed at the surface with <br /> traffic-rated watertight well boxes set flush with the existing surface. <br /> The drilling contractor will be Layne Christensen Company of Fontana, California (C57 <br /> License No. 510011) and their Licensed Contractors Declaration (LCD) has been provided <br /> to your office previously. Underground Service Alert (U.S.A.) will be contacted to field locate <br /> underground utilities prior to commencing the field investigation. <br /> Drilling and well installations for this project are scheduled to begin on December 6, 2004 <br /> and will be completed by approximately February 1, 2005. <br /> Check No. 4682 for$89.00 is enclosed for processing the new well installation permit <br /> application. We trust that the information provided in this letter is sufficient for processing <br /> the new permit application and associated addendum to the existing permit.
