The applicant shall pay for the <br /> costs of road construction. <br /> 18 . The applicant shall retain a certified civil engineer to con- <br /> duct annual inspections and report to the Planning Commission; <br /> such reports are to attest to the applicant ' s compliance with <br /> activities authorized or required under this permit . <br /> 19 . The permit request and site plan shall be revised to include <br /> an asphalt batch plant , and any future cement batch plant <br /> shall be subject to an excavation permit application. <br /> 20 . Use of the proposed truck scales shall be limited to custo- <br /> mers in the process of hauling materials from the site . <br /> 21 . Site access to Waverly Road shall be relocated approximately <br /> 157 feet southerly of existing entrance and shall be <br /> constructed with minimum 50 ' radiuses and 100 ' minimum accel- <br /> eration and deceleration lanes , all subject to approval of <br /> the Public Works Department . The use of other routes for <br /> access to the site is prohibited. <br /> 22 . Prior to the commencement of excavation, the applicant shall <br /> submit for approval by the decision-making body: <br /> a. A revised site plan which shows the site access, locations <br /> of structures , excavation phasing, project boundaries, <br /> paved surfaces, and elevations of excavation depths and <br /> final surfaces . <br /> b. A revised Excavation and Rehabilitation Schedule which <br /> details the locations, acreages, and cubic yardages to be <br /> excavated and rehabilitated and the timetables and sequen- <br /> ces in which these operations shall be accomplished. <br /> c . A revised Rehabilitation Plan which incorporates the <br /> reseeding recommendation of the SCS (Soil Conservation <br /> Service) or the Agricultural Extension Service . <br /> 23 . A plan for the addition of vegetative visual screening shall <br /> be submitted for review and approval by the Planning <br /> Division. <br />
