The following is an itemized list of underground storage tank violations that have not <br /> been addressed for CRUISERS MANTECA#29 as of April 13, 2018. <br /> Open violations from September 07, 2017 inspection <br /> Violation #201 -Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. The 2014 and 2015 monitoring <br /> system certification tests reports were not found on site. These records shall be maintained on site for at least three <br /> years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment <br /> calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed <br /> a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or <br /> other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all <br /> maintenance and monitoring records for the last three years on site and submit copies to the EHD <br /> Violation#204 -Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> Violation#205 - Failure to have an approved UST response plan available onsite. <br /> An approved copy of the response plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> Violation#206 -Failure to comply with UST operating permit conditions. <br /> No person may own or operate an UST unless a permit for its operation has been issued by the local agency to the <br /> owner or operator of the UST system and all the operating conditions have been met. Upon the inspection of onsite <br /> records it was found that work requiring the notification or issuance of a permit by the EHD prior to beginning of <br /> work was performed. On 10/31/2016 work order 808380 indicates that replacement of sensor S7, on 01/16/2017 <br /> work order 2783205 indicates the replacement of the S7 smart sensor, on 01/06/2017 work order 81796 indicates <br /> replacement of the 91 fill brine sensor and notification was not provided to the EHD. Emergency repairs relating to <br /> UST system monitoring and release detection may be performed by a properly licensed, trained and certified <br /> contractor pursuant to the EHD's policy and a permit obtained within one business day from the EHD. All other <br /> repairs must have a permit submitted to and approved by the EHD prior to beginning work. To date, the EHD does <br /> not have evidence of notification for the replaced sensors. A full permit needs to be obtained for the replaced parts <br /> and a permit fee will be charged. <br /> Violation#305 -VPH monitoring of the interstitial spaces of the UST system is not maintained. <br /> The S1 and S6 smart sensors failed when tested. The interstitial space of the UST shall be maintained, as <br /> designed, under constant vacuum such that a breach in the primary or secondary containment is detected before <br /> the hazardous substance is released into the environment. The S1 smart sensor was fixed on site. Correct <br /> immediately by having a properly licensed, trained, and certified contractor replace the failed component with a <br /> functional component(LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If <br /> the failed component can't be replaced immediately, there is a possibility that the UST system monitored by the S6 <br /> smart sensor may be red tagged to prevent fuel inputs. <br /> The UDC 7/8 had very little evidence of brine presence. The service technician added brine to UDC 7/8. <br /> Loss of brine can be indicative of a leak in the secondary containment. Secondary containment shall be impervious <br /> to the liquid and vapor of the substance contained and constructed to prevent structural weakening as a result of <br /> contact with any hazardous substances released from the primary containment. Immediately contact a properly <br /> licensed, trained, and certified contractor to make repairs to the UST system under permit and inspection of the <br /> EHD. <br /> Page 2 of 4 <br />
