Christ Knight removed the item from the shelf and placed it into the <br />pharmacy's HW bin at the time of inspection. <br />All hazardous waste shall be contained in closed container at all times except when adding or removing waste. <br />Immediately ensure all hazardous waste is contained in a closed container and ensure all hazardous waste <br />containers are closed when not adding or removing waste. <br />This was corrected on site. <br />This is a Class II violation. <br />605 <br />CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br />A box of Rite Aid Pharmacy Nicotine Lozenges with an expiration date of 09/18 was found for sale on a shelf behind <br />a front store register (IMG 2111-2112). Christ Knight removed the item from the shelf and placed it into the <br />pharmacy's HW bin at the time of inspection. <br />All hazardous waste containers shall be marked with the following information: <br />- the words "Hazardous Waste" <br />- name and address of generator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />Immediately label ensure all hazardous waste containers are marked with all the required information. <br />This was corrected on site. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />NOTES: <br />- At the time of inspection, a pharmacy technician, Chong Chang, was asked how expired bottles of warfarin or <br />coumadin are managed. Chong stated she couldn't remember a time where the pharmacy had any expired <br />warfarin or coumadin since it is used by consumers rapidly, however she stated that if they were to have expired <br />warfarin or coumadin it would be sent back to Med Turn., Inc. Provide the EHD with Rite Aid policy and <br />procedure for how expired warfarin or coumadin are managed. <br />An inspection checklist was provided to the facility operator on the day of inspection.
