Main St. CUF#: 12540 <br /> Stockton, CA 95202 Global ID#: T607700430 <br /> In a letter dated November 10, 2008, the San Joaquin County Environmental Health <br /> Department (EHD) directed that an EHD Well Permit Application Form and fee be submitted by <br /> November 24, 2008, and that remediation wells be installed by December 10, 2008. The permit <br /> application and fee has not been received and the remediation wells have not been installed as <br /> directed. <br /> On December 16, 2008, the EHD received a letter dated November 24, 2008, from Mr. Thomas <br /> O'Neill. In the letter it discusses that the proposed work cannot be performed because of a lack <br /> of funds and claims that reimbursement for previous costs have not been received from the <br /> State Water Resources Control Board (SWRCB) Clean-Up Fund (CUF) since July 2007. <br /> The EHD contacted the CUF manager at the SWRCB to discuss this matter and learned that <br /> claims #11 and #12 totaling nearly $100,000 have been paid out to the claimant since <br /> November 2006. Only claim #13, submitted to the CUF on December 5, 2008, is outstanding. <br /> In order to discuss this matter and the failure to comply with the EHD directives, your presence <br /> is requested at a meeting to be held at the EHD offices, 600 E. Main Street, Stockton, on <br /> February 5, 2009, at 10:00 a.m. The Central Valley Regional Water Quality Control Board <br /> (CVRWQCB) will also be in attendance at this meeting.
