Main St CUF#: 12540 <br /> Stockton CA 95202 Global ID#: T0607700430 <br /> In a letter dated November 10, 2008, the San Joaquin County Environmental Health <br /> Department (EHD) directed you to submit a Well Permit Application Form and fee by November <br /> 24, 2008 and to install the remediation wells by December 10, 2008. The permit application and <br /> fee has not been received and the remediation wells have not been installed as directed. <br /> On December 16, 2008 the EHD received your letter dated November 24, 2008. In this letter <br /> you claim that you can not afford to enact the approved remediation well installations because <br /> you have not received a State Water Resources Control Board (SWRCB) Clean Up Fund (CUF) <br /> reimbursement since July 2007. <br /> The EHD contacted the CUF manager at the SWRCB to discuss this matter and learned that <br /> only claim #13, submitted to the CUF on December 5, 2008 is outstanding. Per the CUF, claim <br /> #12 for $28,000 was approved and you were notified in correspondence dated November 18, <br /> 2008 of the pending payment. <br /> In order to discuss this matter and your failure to comply with the EHD directives, your presence <br /> is requested at a meeting to be held at the EHD offices, 600 E. Main Street, Stockton on <br /> January 8, 2009 at 10:00 a.m. The Central Valley Regional Water Quality Control Board <br /> (CVRWQCB) will also be in attendance at this meeting.
