The EHD letter required you to submit the Well Permit Application Form <br /> and fee immediately. As of the date of this letter, the application and fee have not been <br /> submitted. <br /> You are hereby directed to submit a Well Permit Application Form and $404 fee by <br /> October 17, 2008. , <br /> You are hereby directed to have the two remedial wells installed and tested by December <br /> 10, 2008. <br /> You are hereby directed to submit the report of findings for the well installation and the <br /> results of the remedial pilot test to Geotracker by February 10, 2009. <br /> The EHD advises you that responsible parties are required to be in compliance with directives <br /> issued by the EHD.
