The 9-5/8" casing is cemented with sufficient cement to fill behind this casing <br /> from the shoe to the ground surface. <br /> 2. The 5-1/2" casing is cemented with sufficient cement to fill behind this casing <br /> from the shoe to at least 500 feet above the uppermost oil or gas zone. <br /> 3. Drilling fluid of a quality and in sufficient quantity to control all subsurface <br /> conditions in order to prevent blowouts is used. <br /> 4. Blowout prevention equipment conforming to Class III B 2M requirements is installed <br /> on the 9-5/8" casing and maintained in operating condition at all times. See <br /> Manual M07 and attached sheet. <br /> 5. Blowout prevention equipment conforming to Class "III" 2M requirements is installed <br /> on the 5-1/2" casing and maintained in operating condition at all times. See <br /> Manual M07 and attached sheet. <br /> 6. Fresh water protection for this well shall be as shown on attached exhibit A. <br /> 7• The division is furnished copies of any draw down or back pressure tests performed. <br /> In addition the division will monitor the production for a period of six months and <br /> if anomalous water production is indicated, remedial action will be ordered. <br /> 8. THIS DIVISION IS NOTIFIED: <br /> a. Before deviating from the proposed casing program and/or placing any plugs in the <br /> hole. Additional requirements shall be outlined at that time. <br /> b. To witness a test of the installed blowout prevention equipment prior to drilling <br /> out the shoe of the 9-5/811 casing. The blind blank rami <br /> o erator g / shall be '-c�t�d '-;� -- <br /> operator prior to calling the Division inspector. Test information of the blind/ <br /> blank ram test shall be entered on the tour sheet along with the signature of the <br /> person in charge. <br /> NOTE: 1.
