4) 415 N. Main Street (Subject Site): <br /> The San Joaquin County Public Health Services, Environmental Health Division <br /> files for the subject site were reviewed. The following information was obtained: <br /> Prior to ATEC's Phase II subsurface investigation, ATEC was required to submit <br /> a Site Mitigation Request, Workplan, and Health and Safety Plan. These forms <br /> are dated November 4, 1992 and are the latest file correspondence available <br /> regarding the subject site. <br /> The file also contained a request, dated September 4, 1991, from Artesian <br /> Environmental Consultants of Mill Valley, California to perform a Phase II <br /> investigation on the site. The owner of the property is listed to be Chatham <br /> Capitol Management Associates (this institution is also referenced as the current <br /> building owner in the May 1991 Phase I Report). The file does not contain a <br /> proposed scope of work. However, an application for a Permit to Work dated <br /> September 3, 1991 completed by Artesian Environmental indicates that soil <br /> borings will be drilled at unspecificed locations on the property and additional <br /> work could be performed depending on the results of a geophysical survey. The <br /> file does not contain any indications that this proposed work was ever performed; <br /> however, the form is stamped which indicates receipt of Artesian's payment of <br /> required EHS fees for the site work. <br /> 14 <br />
