C A M B R I A Ms. Gori Duncan <br /> November 18,2005 <br /> Site Health and Safety Plan: The general contractor will be required to prepare a comprehensive <br /> site safety plan to protect site workers. The plan will be kept on site during field activities and <br /> will be reviewed and signed by each worker and all visitors to the site during construction <br /> activities. Cambria will prepare a site safety plan to protect its employees during oversight <br /> construction work. <br /> Construction: Cambria will provide oversight of construction activities included in the <br /> contractor's scope of work. The contractor will arrange all required inspections. The schedule to <br /> install the DPE system is contingent on issuance of all applicable permits and the contractor's <br /> availability. <br /> © DPE System Operation and Maintenance <br /> Site Health and Safety Plan: Cambria will prepare a site-specific health and safety plan for <br /> routine operation and maintenance activities. The plan will be kept on site and signed by <br /> Cambria's technician each site visit, by any subcontractors performing work on the DPE system, <br /> and by any visitors or inspectors entering the established work zone. <br /> Start-up: Start-up of the DPE system will be conducted after final inspection approval and in <br /> accordance with the SJVAPCD permit and waste discharge permit. The SJVAPCD typically <br /> requires analysis of the inlet and exhaust streams within the first 10 days of operation to confirm <br /> compliance with the total flow rate, benzene emission limits, and constituent destruction <br /> efficiency requirements. Cambria will collect wastewater discharge compliance samples and <br /> submit results in accordance with the OMI wastewater discharge permit requirements. A <br /> summary report of start-up activities will be submitted in accordance with permit conditions. <br /> Data Collection and Optimization: Cambria anticipates conducting semi-monthly operation and <br /> maintenance site visits. Data will be collected on site-specific standard forms (Attachment D). <br /> During each site visit, Cambria will record the operational status, hour meter reading, <br /> groundwater flow rate, and flow totalizer meter reading. The depth to water in on-site DPE wells <br /> and monitoring wells will be measured periodically to verify drawdown in the extraction wells <br /> and to asses the level of hydraulic control achieved via DPE. The DPE stinger depths may be <br /> adjusted periodically to maximize hydraulic control and/or dewater certain areas beneath the site <br /> for more effective SVE. <br /> Cambria will monitor pump vacuum, system vacuum, well vacuum, system flow, dilution air <br /> flow, well flow,and system temperatures to assess Solleco unit operation. A thermal anemometer <br /> will be used to measure extraction flow rates. Cambria will monitor vapor concentrations <br /> entering and exiting the Solleco unit to evaluate destruction efficiency and permit compliance. <br /> Field vapor concentrations will be measured with a Horiba model MEXA554JU organic vapor <br /> analyzer(OVA)or equivalent instrument.
