PHS/EHD has the following comments for your
consideration.
As PHS/EHD indicated in correspondence dated August 12, 1994, a closure report prepared
according to the Appendix B of the Tri-Regional Guidelines should be submitted for
PHS/EHD review. In review of the closure report that was submitted, PHS/EHD has
identified and summarized a number of the issues which require clarification. PHS/EHD has
already communicated these problems to your consultant, Golder Associates.
The report did not indicate that it had been reviewed by a Registered Professional.
- Boring logs from MW8 and MW9 were not included. Soil sample results from MW9
were identified as EX I rather than MW9.
There was a notation in the report that a soil gas survey occurred in May 1990 when
actually the soil gas samples were collected on July 3, 1990. A map with the sample
locations was included without the analytical results.
- The report indicated that numerous wells were destroyed during remediation. MW4S
was the only well destroyed; however, MW4D was damaged.
- The summary of tank removal results contained errors for the samples collected from
beneath the 1,000 and 3,000 gallon tanks.
- There were analytical results from A, B, and C drums which were not referenced.
- The soil and groundwater sample results which were obtained from MW10 - MW12
were not included.
A Division of San Joaquin County Health Care Services
