PHS/EHD has the following comments for your <br /> consideration. <br /> As PHS/EHD indicated in correspondence dated August 12, 1994, a closure report prepared <br /> according to the Appendix B of the Tri-Regional Guidelines should be submitted for <br /> PHS/EHD review. In review of the closure report that was submitted, PHS/EHD has <br /> identified and summarized a number of the issues which require clarification. PHS/EHD has <br /> already communicated these problems to your consultant, Golder Associates. <br /> The report did not indicate that it had been reviewed by a Registered Professional. <br /> - Boring logs from MW8 and MW9 were not included. Soil sample results from MW9 <br /> were identified as EX I rather than MW9. <br /> There was a notation in the report that a soil gas survey occurred in May 1990 when <br /> actually the soil gas samples were collected on July 3, 1990. A map with the sample <br /> locations was included without the analytical results. <br /> - The report indicated that numerous wells were destroyed during remediation. MW4S <br /> was the only well destroyed; however, MW4D was damaged. <br /> - The summary of tank removal results contained errors for the samples collected from <br /> beneath the 1,000 and 3,000 gallon tanks. <br /> - There were analytical results from A, B, and C drums which were not referenced. <br /> - The soil and groundwater sample results which were obtained from MW10 - MW12 <br /> were not included. <br /> A Division of San Joaquin County Health Care Services <br />
