(Apex) on your behalf. <br /> In the work plan, Apex proposes to destroy eleven monitoring wells (MW-1, MW-2, MW-4A, <br /> MW-5, MW-6, MW-9, MW-12 through MW-14, MW-16 and MW-17), and ten ozone sparging <br /> .r wells(OS-1 through OS-10) by pressure grouting. Apex proposes to use a tremie pipe to grout <br /> to the surface with portland cement slurry followed by applying 25 pounds per square inch (psi) <br /> of pressure for a minimum of five minutes, and then Apex over-drilling the top five feet and <br /> backfilling the hole with portland cement slurry to within a foot of the ground surface. <br /> Apex also proposes to destroy five monitoring wells (MW-3, MW-10, MW-11, MW-15 and MW- <br /> 18), two monitoring/vapor extraction wells (MW-7NEW-4 and MW-8NEW-3), two vapor <br /> extraction wells (VEW-1 and VEW-2), and one recovery well (RW-2) by over-drilling the wells to <br /> .r total depth using augers with diameters equal to or greater than the augers used to install the <br /> wells; then, Apex proposes to grout the borehole with portland cement slurry using a tremie <br /> pipe. <br /> The EHD has reviewed the historical soil and groundwater analytical data for this site. Based <br /> on the presence of petroleum hydrocarbon contamination in soil samples collected at the time of <br /> well installation, or the location of the well near known or suspected contamination, or the <br /> detection of contaminants of concern in relatively recent groundwater samples collected at the <br /> site, the EHD approves the following: <br /> r <br /> r <br />
