Former Moore Petroleum Bulk Plant Page 2 of 3 <br /> 5491 F Street December 26, 2012 <br /> Banta, California 95304 <br /> Over-drill monitoring wells MW-1, MW-2, MW-3, MW-4A, MW-5, MW-7NEW-4, <br /> MW-8NEW-3, MW-9, MW-10, MW-12, MW-15, and MW-18; ozone spare wells OS-1, <br /> OS-2, OS-3, OS-6, OS-7, 0", and OS-10; vapor extraction wells VEW-1 and VEW-2; <br /> and recovery well RW-2. <br /> y • Pressure grout monitoring wells MW-6, MW-11, MW-13, MW-14, MW-16, and MW-17; <br /> and ozone sparge wells OS-4, OS-5 and OS-9. <br /> When pressure grouting each well, apply 25 psi of liquid pressure for at least ten minutes, then <br /> drill out the top three feet or more of the well with an auger that is at least one foot larger in <br /> diameter than the auger originally used to install the well. The well casing should be cut off six <br /> inches above the bottom of the three-foot hole, allowing the grout sealant to spill out of the top <br /> of the well casing and form a "mushroom cap" seal with at least twelve inches of neat cement <br /> grout poured six inches above and six inches below the top of the cut-off casing. <br /> Monitoring well MW-18 was constructed with a steel conductor casing from approximately fifty <br /> feet below surface grade (bsg) to surface- The conductor casing may remain in place; however, <br /> the two-inch diameter Schedule 40 polyvinyl chloride (PVC) casing, filter pack, and grout <br /> between the PVC casing and metal conductor casing must be removed and the borehole filled <br /> under pressure from total depth of approximately 80 feet bsg to surface with 10.3 Sack <br /> sand/cement grout, forming a "mushroom cap"at approximately three feet bsg. <br /> All groundwater displaced to the surface during grout installation must be pumped out of the <br /> excavation to allow for the EHD inspection and verification that the well has been properly <br /> destroyed. Contaminated groundwater pumped out of the excavation must be containerized for <br /> proper disposal. <br /> This site has seven wells located off-site in the San Joaquin County right-of-way and at least <br /> one well on the Union Pacific Railroad Company property. Please submit a copy of the valid <br /> encroachment permit issued by the County and a copy of the access agreement from the Union <br /> Pacific Railroad Company when submitting the EHD Well and Boring Permit Application form for <br /> these wells. <br /> The EHD approves the work plan with these conditions. Please submit the EHD Well and <br /> Boring Permit Application form for each parcel, Master File Record (MFR) form, Encroachment <br /> Permit issued by the County of San Joaquin Department of Public Works, and the appropriate <br /> fees for each parcel. The permit fee for the destruction of each well is $125.00, along with a <br /> $375.00 Inspection Activity fee per parcel. A review of the site records indicates that 31 wells <br /> currently are installed. A fee of $5,000.00 (31 wells x $125.00 + $375.00 x 3) will be required <br /> for the well destruction permits. <br /> r <br /> Please provide the EHD certification in writing that all record owners of fee title to the above- <br /> referenced site have been notified of your request for closure. This is in accordance with <br /> Section 25297.15 of the California Health and Safety Code. Submit a complete list of record <br /> owners of fee title and their mailing addresses to the EHD. If you are the sole landowner, <br /> provide a statement to this effect to the EHD. A form has been enclosed for this purpose. <br /> r <br /> v <br /> V <br />
